Lifetime Achievement Award Application Form

This prize fund is to recognise a veterinarian or scientist who has dedicated much or most of his/her career to advancing the health and welfare of dogs. The individual is likely to have made a significant impact on the world stage of canine health. The Application requires a Nominator and four Seconders. The Prize Fund offered for this award is £10,000.
